Nurse Diane Lake

Nurse Sian Smith
RGN, Advanced Diploma, Independent Prescriber
Diane and Sian are both Nurse Practitioners. They are qualified and registered nurses and can help with health issues such as family planning, healthy living advice, blood pressure checks, travel clinics, baby clinics and dressings. The nurses also run clinics for long-term health conditions such as asthma, diabetes, COPD and Coronary Heart Disease for which annual review and monitoring will take place, minor ailment clinics, for which they can prescribe. All nurses carry out cervical smears.
Our Nurses work closely with the Doctor and can refer to other health and social workers when required.
Healthcare Assistants
Michelle Derbyshire
Michelle is our healthcare assistant is available to help with things such as blood pressure monitoring, healthy heart checks, B12 vaccinations and phlebotomy. She holds health care clinics on Wednesday, Thursdays and Fridays.
General Practice Assistant
Karen Henderson
Karen is our General Practice Assistant. She is available for healthcare clinics such as phlebotomy, blood pressure checks, height and weight and also helps the GPs with administrative tasks such as referrals and correspondence.
Practice Management
Sue Cullen
Sue is our practice manager and has worked at the practice for over 20 years. Sue will be able to help you with any administrative problems or concerns you may have. She is responsible for dealing with any complaints you may have about the surgery. If you wish to make a complaint you can speak to any member of our team who will be happy to help you or if you wish you can speak to Sue who will be happy to discuss your concerns. She will also be very happy to receive any compliments, comments or suggestions you may make to improve our services. In her absence our Assistant Practice Manager, Nicola Sumpter, will be happy to help.
Reception
Our Reception and Admin team comprises Elaine, Janet, Janice, Wendy and Debbie. They answer the phone, deal with enquiries and take repeat prescriptions as well as a host of other administration tasks. Their job is very demanding so please be patient. They work within the practice guidelines and follow the practice protocols, please remember they are carrying out doctor’s instructions.
Community Nurses
The District Nursing teams work in conjunction with the doctors. They are able to offer a comprehensive nursing service. Home nursing care, support to carers, care of the elderly and advice. Assessment is always made by a qualified district nursing sister. They can be contacted by telephoning the surgery and leaving a message with a member of the reception team who will contact them.
Health Visitors
Our health visitors team are trained nurses promoting good health to our patients in the community. They meet with patients in their own homes. The Health Visitor will advise on many aspects of health, particularly child health, development and behaviour, they can be contacted via the surgery by passing a message to the reception team who will contact them.
